Before Your Appointment
Please complete all requested forms prior to your visit whenever possible.
Patients may also be asked to bring:
- Insurance card(s)
- Photo identification
- MRI, CT, X-ray, or other imaging discs and reports
- Referral or authorization forms if required by insurance
- A current medication list
Bringing completed paperwork and imaging studies helps Dr. Garges review your condition more efficiently during your spine evaluation.
